- If alien employee is already in the U.S. copies of: passport, I-94 and documentation of current immigration status (DS 2019, I-797 A/ H-1B. Approval Notice, Optional Practical Training Employment Authorization Document/EAD, J-1 Approval Notice, copies of recent pay stubs, etc.)
- Two letters of recommendation addressing professional competency
- Pay stubs for two consecutive months (for H-1B portability only)
- For physician positions: WV medical license, ECFMG Certificate, USMLE Scores for steps 1, 2 and 3
- NOTE: If required documents are not in English, they must be translated and certified by a person competent in both the source language and English. Translations need to be notarized.
III. *FROM ALIEN EMPLOYEE’S DEPENDENT IF CURRENTLY IN THE U.S.
- I-539 USCIS Form completed by the alien employee’s dependent (not the alien employee). The form can be accessed at: http://www.uscis.gov
- $300.00 personal check made out to U.S. Citizenship and Immigration Services (USCIS) paid by the alien employee
Important: Please note that credit cards and cashier’s checks can not be used for paying this fee. - Copies of passport and I-94 documents
- Evidence of relationship to the principal alien employee, such as marriage certificate or birth certificate

* Basic Fee: $320.00 payable to United States Citizenship and Immigration Services (USCIS)
Anti-Fraud Fee: $500.00 payable to U.S. Department of Homeland Security (one time fee)
Note: It applies to all new H-1B petition including H-1B portability cases.
* Premium Processing: $1,000.00 payable to United States Citizenship and Immigration Services (USCIS). This fee is to be paid in addition to the basic fee for cases requiring expeditious adjudication.
Important: Please note that Procurement Cards (PC) cannot be used for paying the above fees.
