Notes:
- Most forms that you need to fill out are FILLABLE .pdf documents. Just click your cursor where you need to put information, then type and print it. You may tab between fields. You need at least Acrobat Reader 5.0 to use these files. It is a free download.
- Please allow us two business days to process your request for any letter.
General Information
- It is VERY important that you notify the OISS of your address whenever you move. To do this, complete Form AR-11(amended) and drop it off in our office if you are in F-1 or J-1 status, or Form AR-11 if you are in another visa status.You can check the status of your application that has been sent to the USCIS for processing on their website!
* If you are a new student or scholar, or if you are a continuing student or scholar but we don’t have a file for you, please fill in the Contact Information Form and then print it, and bring it to our office.
* If you are a graduate of WVU, we’d like to keep in touch (and if you’re on OPT we are required to keep in touch)-you can submit your new location to us by email or use our online form -please include your full name, address, phone number, and email address..
* Submit a form directly from our website!
o Request for a Letter of Good Standing (required for a driver’s license and sometimes by other organizations)
o Request for a Visit Letter (inviting friends or relatives to visit you here)
o Volunteer Form—to let us know your interests so we can help you find ways to connect locally.
o Form I-539 is the application form for change of status, B-visa extensions, reinstatement, etc. * Use I-20/DS-2019 Request Form
o for replacement of a lost I-20/DS-20
o to bring your family to join you.
o Also required when applying for I-20 extension.
o Do NOT use this form to request an initial form or for a J-1 extension.
