- Addendum to CV (template) – See U.S. Permanent Residency.
- Agreement (Requirements) – Occasionally, review the agreement you signed, which verifies that you know the terms of your visa status. Being a student with F-1 or J-1 status, you are responsible for learning, understanding, and complying with the U.S. laws and regulations that apply to you. If you fail to do so, you could be deported from the United States and barred from returning for an extended period of time, thereby jeopardizing your academic career.

- AR-11 Form – SUBMIT your current local address to the OISS using Form AR-11 (amended) within ten (10) days after you move so that it can be reported through the SEVIS system, as is required by law. We will also update your contact information with Admissions & Records. Immigration regulations require you to always advise OISS and the Office of Admissions and Records regarding any changes in your Morgantown address, phone number, and e-mail address within 10 days of any new changes. In order for you to receive your mail, your grades, and tax information, you must tell the above offices. An AR-11 Form needs to be submitted to OISS to document the change.

- DS-2019 Request – department must execute the attached Request for J-1 Visa Document and send it to the Office of International Students and Scholars. The request must be accompanied by a copy of the letter of invitation, specifying the visitor’s duties and the source of funding during the visitor’s stay.
- Driver's License (Applying for a West Virginia Driver's License) – Please plan to arrive at the DMV Office before 2:00 pm for testing and licensing because of the additional time required to verify documents and legal presence status, check with Help Desk, enter the unique license term information to sync license expiration and length of legal stay and the longer period of time it will take you to complete the testing.

- Evaluation Form: International Orientation – if you participated in the most recent International Orientation, you can help the next group of international students by telling us what was helpful for you and what could be improved to be more helpful to new international students.
- Exit Form – If you are graduating and do not plan to return to WVU or the United States, please fill out the exit form and submit, so we can give you a COMPLETED in SEVIS. It is important for your future travel that your status in the SEVIS database receives this update.
- Extension of I-20 or DS-2019 – Bring NEW financial documentation when you need a new I-20 or DS-2019 or for an extension if your current form will expire within two months and you will not complete your program in that time.

- Health Insurance Requirement – All international students are required to have health insurance the entire time that they are in the US to attend school or work on OPT with an I-20 or DS-2019 from WVU. If you are a current student at WVU, you MUST show verification of your health insurance coverage (the policy) to Carolyn Matthews at the Student Health Service, unless you have the WVU-sponsored policy (United Health Care), as soon as the new Semester begins.Your insurance papers MUST be in English and show your coverage in US dollars. If you do not do this, your records can be put on hold, preventing you from registering for classes. - I-539 Form (Application to Extend / Change Nonimmigrant Status) click here for current form.
- Immunizations Requirement – West Virginia University now requires that all new incoming international freshmen receive two (2) doses of the Measles, Mumps and Rubella vaccine (MMR) and one dose of meningitis vaccine before entering WVU. All transfers must have the two (2) doses of MMR. For more information, please contact the WVU Admissions and Records Office.

- J-1 Termination Report Form – You must complete the attached Termination Report and forward it to the OISS within 10 days before the expiration of the current DS-2019 form. In some cases, the visitor may be transferring to another institution to continue the same activity. If this is the case, the visitor will need to notify the OISS of the program they intend to transfer to, and the intended transfer date, at least 30 days before his/her intended transfer date or current DS-2019 expiration date, whichever comes earlier. - Less Than Full Time Permission – ENROLL in more than the minimum full-time course load (12 credit hours undergrad; 9 for grad) so that if you drop a course, you will still have a full-time course load to satisfy your F-1 or J-1 non-immigrant status. If you intend to drop below full-time, you MUST come to our office BEFORE you drop any classes or register for less than the minimum hours for ANY reason.

- State Identification Card – State-issued identification cards are a handy resource if you do not, or are not old enough to, carry a driver’s license. The OISS has gathered the information you’ll need to apply for an ID in your state.

- Tax Returns – West Virginia University has licensed income tax return preparation software for use by international students, scholars, teachers, researchers, trainees. This new software is provided free of charge to aid them in preparing U.S. federal income tax forms. It is designed to take the time and confusion our of filing a U.S. tax return – it takes only about 20 minutes to complete, print and file a tax return. Please read the Instructions and the FAQs thoroughly before proceeding with your tax returns.
- Transfer in Form – for International Students transferring from other universities to WVU.
- Transfer out Form – for International Students transferring from WVU to universities.
- Travel – Please remember to bring your I-20 to the OISS at least two weeks before you leave the U.S. so that we can make sure that your I-20 has been signed within a year of your intended return to the U.S.
- Tuition Merit Award FORM – A tuition waiver offered to qualifying undergraduate international students will be available February 2010. If you have financial need and good grades, and you are in at least your second semester at WVU, you may apply for this award.

- Visit Letter – Family members of International students may enter the United States using a B Visa or ‘Visitor for Business or Pleasure’ Visa. This Letter is to facilitate the Visa application procedure through the U.S. Embassy process of B Visa Application.
- Volunteer Interest Questionnaire – We want to give you an opportunity to enhance your awareness of the Morgantown community by volunteering to participate in one or more programs organized by the OISS. It has been our aim to invite community organizations to sponsor a variety of activities that we have throughout the school year. As these organizations continue to sponsor ongoing events, we in return try to promote a healthy interchange between the community and you the student. Though this appeal is primarily intended for students, we are happy to accept volunteers who are here as researchers, scholars, family members, etc.
